- Domicile
- DMC of last exam passed.( 2 copies)
- Character Certificate
- Form-B
- Father CNIC & Mother CNIC
- Five passport size picture
- Online application form in hard form.
- Sports certificate (for sports quota)
- Disability certificate (for disable quota)
- Parents employment certificate from HED (for HED employees only)
- Migration certificate(Other Board)
- Stamp paper from court for admitting that he had not taken admission in any other college ( Affidavit)
- Afghan (approval of documents from afghan commissioner)
